- I am aware that the CCCSL Coed 16” Softball
League is a hazardous activity that involves the risk of serious injury and even
death and that not all hazards or risks arising out of, connected with, or in
any way associated with my participation in this activity can be foreseen.
- I am at least 21 years of age or older and I am voluntarily
participating in this activity with knowledge of the dangers involved and agree
to voluntarily assume the risks of any and all injuries and liability arising out of,
connected with, or in any way associated with my participation in this activity.
- I agree that I am solely responsible for determining
whether or not I am physically fit and/or skilled for the activities
contemplated by this agreement. I understand that
it is always advisable, especially if I am pregnant, disabled or have suffered
or am suffering from an illness or an impairment, to consult a physician before
undertaking any physical activity.
- I, for myself, my heirs, administrators, executors,
administrators and assigns, waive and relinquish any and all claims, demands,
actions or causes of action which I may have against CCCSL, NFP, its officers,
directors, employees, agents, representatives, or volunteers, any league
sponsors or organizers and any of their predecessors, successors, heirs,
affiliates or assigns (“Released Parties”), for any injuries, damages, liability, expenses or
losses of any kind to my person or property arising out of, connected with, or
in any way associated with my participation in this activity. I also agree to
indemnify and hold harmless the Released Parties for any and all claims,
demands, actions, causes of action, injuries, damages, liability, expenses or
losses of any kind arising out of, connected with, or in any way associated with
my participation in this activity, including but not limited to any attorneys’
fees which I incur as a result of any of the foregoing.
